Registration Administrative

1.      Ophthalmologist and attach the sertificate of study

2.       Copy of Current Practice Licence

3.       Curiculum Vitae

4.       Color Photo 4x6 (4 exp.) and 3x4 (2 exp.)

5.       Fill the registration form

6.       Fill the contract study form

7.       Recommendation Letter from 2 ophthalmologists, complete with the mailing adress

8.       Experience in eye surgery
